Stop tracking .env, add .gitignore

.env should never be committed. It stays on disk untouched (contains
DB credentials and the new IMMICH_TRUSTED_PROXIES setting) but Git no
longer tracks it. Also picks up an unrelated pre-existing GPU device
tweak in hwaccel.ml.yml.
